Renal Function

 

Adverse drug reactions may occur when medication dosage is not adjusted for renal function. A standard formula, Cockcroft-Gault Equation, may be used to calculate the patient's creatinine clearance for patients 18 years of age and older. Consult a medical reference to determine the appropriate dose for the patient's degree of renal dysfunction.

Creatinine clearance in ml/min [Cockcroft Gault Equation] 
    Males =  [(140-age) x lean body weight] / (72 x serum creatinine)
    Females = 0.85 x males

Lean body weight (kg)
    Male = 50 + 2.3 x (height in inches - 60)
    Female = 45.5 + 2.3 x (height in inches - 60)
